The course is a 2.2‑mile loop through the Maryville College Woods with approximately 120 feet of elevation gain per loop.

All participants must follow event rules as provided by the organizer. The aid station is reserved for runners and pacers only (non‑runners may not access it). Pacers must pick up a bib at packet pickup and wear it while pacing. Runners may set up personal aid areas under McArthur Pavilion or in the large yard around it. Chip timing is used; bibs must be worn on the front (not back) and not folded, especially around the chip.
The Three Sisters are a close‑set group of peaks on Chilhowee Mountain south of Maryville, visible from the higher parts of the Maryville College campus. The event is named after these peaks. The course record (as of 2021) is 121.875 miles (56 loops) by Luke Bollschweiler.
